appearance
Soho Reina is a beautiful woman with refined, elegant features. Her nickname of "Lady" is certainly appropriate in regard to both her poise and physical exterior. Reina is not particularly tall (she's only 165cms in height) nor does she have any truly exaggerated physical features (unlike the outrageously buxom Shiori). Her traditional elegance draws an intentional contrast to Masane's somewhat wild and outrageous exterior.
general appearance
Reina has long dark hair with a hint of purple to it. Her eyes are a dark brown in colour and her body shape is fairly petite. All of these features are mirrored in her two biological daughters (Rihoko amd Maria). Both daughters also have naturally dark, purple-looking hair with brown eyes and slim figures (though for much of the series, Maria's hair is dyed). Reina has narrow eyes and a perpetually down-turned mouth. These features combine to give her an overall look of sadness. She wears simple red-stud earrings which match her attire and lipstick. When working, Reina ties her hair up in an ornamental gold clasp.
In the third DVD booklet, Uno Makoto mentions that Reina was one of the first cast members that he designed for the series. He also notes his dismay at losing her character so early in the story:
Both Shiori and Reina end up dying pretty quickly (laughs). I would have liked them to have more screen time, just because I agonized over their designs so much, but the story goes fast and I suppose it is better for them to be missed as they vanish from their fleeting lives. Their voices are great too.
regular attire
Reina wears only one real civilian outfit during her portion of the Witchblade series. It is a maroon top and skirt combo with a high collar, short sleeves and a flowing split skirt. When she is at work, she is frequently shown wearing this outfit under her white lab coat. Sometimes she substitutes the maroon top with a slightly lower-cut blue number (as depicted when Shiori is hospitalised). With all variations on her regular attire, Reina wears stockings capped by brown, strappy pumps.

As with everything else about Reina's character, her clothing is functional and little else. She never really highlights her outfits with any personal touches.
bed time reina
Reina's nightwear is only glimpsed briefly in episode fourteen ("Family") after Rihoko moves in to share her apartment. Fitting with her honest and simply presented personality, Reina's nightwear is similarly straight-forward. She wears a basic pajama set with a long-sleeved top and matching knee-length shorts. Both top and bottom are coloured a pale mint-green and have no decorative embellishments or distinguishing features.

Rihoko wears a matching child-sized set of pajamast in pink when she stays with Reina. This outfit is slightly more decorative than her mother's — it has a pink ribbon.
cloneblade form
As with the other advanced Neo-genes in the series, Reina has Cloneblade form. Reina is considered the most powerful of the original generation Neo-genes (as Nora quite openly states) and her Cloneblade form radiates this power. When Reina is "Lady" her body is sheathed with white, near-transparent body armor. It is highlighted all over by traces of red with a single blue line down her face from her eye (like a tear streak)
Lady's body armor extends from her feet through to her hair. The most distinct blade on her body is the one attached to her (left) Cloneblade wrist, though her hair appears particularly formidable as well. It should be noted that Lady's bizarre hair isn't just for show, it can be used as a razor-sharp weapon, just like the actual blade on her wrist.

In the third DVD booklet, Uno Makoto briefly mentions his inspiration for Reina's Cloneblade form. When asked whether or not the post-transformation designs were drawn from specific weapons, he replied:
...blade-related concepts were used for the enemy characters (as well), with sharp scalpels for Reina, a scythe for Shiori and a rapier-like blade for Nora.
Reina, being a doctor, is suited to wearing a weapon designed to look quite specifically like a scalpel. As Reina's skin turns white when she transforms, her outfit wasn't censored for Japanese television broadcast like Masane or Shiori's.
cloneblade
The Cloneblade on Reina's wrist is not particularly notable in appearance, as it looks almost identical to every other Cloneblade shown in the series. Cloneblades are built to resemble the Witchblade in both form and function, though they are not nearly so wild or organic-looking. The key differences being that the Witchblade is rounded in appearance (while the Cloneblades are jagged) and the Cloneblades have blue jewel insets (contrasting with the red one seen on the Witchblade).

When activated, Reina's Cloneblade transforms her into "Lady". All equipped Neo-genes wear their Cloneblades constantly — Cloneblades, like the Witchblade, can only be removed upon their wielder's death.
